The hospitalised bouncer had his teeth and nose broken as well as suffering a nasty cut to the back of his head.
A second was left with cuts over his body and a bite mark on his hand.
The violent clash took place in a car park at a ritzy beach club in North Kuta on Tuesday night at around 9.40pm local time.
One of the tourists who kicked off the scrap is believed to be an Australian with the other nationalities yet to be announced.
Police allege the Australian man harassed a woman inside the club before he was booted out and later turned on security.
Second Inspector Putu Sukarma said: “The security there saw one of the alleged perpetrators strangling another guest sitting at another table.
“The cause was allegedly a row with a woman there.
“The incident ignited emotions. An argument was unavoidable until [the Australian] strangled another customer at the table.”
Club security say they tried to break up the fight after previously warning the man over anti-social behaviour.
Police then allege the Australian was marched out of the club by security guards before managing to break free and start assaulting them.
Four other men followed the Aussie out and also got involved in the scuffle, according to cops.
Footage showed a shirtless man grabbing a security member as another man started to hit him what looked like a bollard.
In the clip, the men could be heard saying “let’s go”, before fleeing the scene shortly after.
The fight even caused traffic outside the club to be interrupted.
Inspector Sukarma confirmed the men are still on the run.
He said: “No one has been arrested related to the incident so far.
“[The] investigation is underway.”
Police said security staff suffered a number of serious injuries in the confrontation.
